The Avaya Healthy Decisions Wellness Center makes it easier than ever to reach your health goals. This easy-to-navigate, secure website is a one-stop shop for better health. Here, you’ll find the tools, trackers, guidance and reliable information you need to take steps toward living a longer, healthier, happier life.

Once you log in to the Wellness Center, you will be able to:

  • Take the Personal Health Assessment to determine your current health status, develop a plan for making changes and track your progress.
  • Track up to ten key health numbers like your blood pressure, blood sugar, calories, weight/BMI, cholesterol, and many more.
  • Participate in online wellness programs.
  • Search for reliable, up-to-date information on thousands of health and wellness topics.
  • Stay up-to-date on medical breakthroughs.
  • Get trusted information on what your symptoms could mean—and what you can do about them.
  • Manage your favorite health-related sites.
  • Get convenient and confidential advice from a healthcare professional by phone or web.

Who can access the online Wellness Center?

  • All active Avaya employees,
  • Family members (ages 18 and over) of active Avaya employees who are covered by an Avaya-sponsored health plan, and
  • All retirees who have primary medical coverage provided by an Avaya-sponsored health plan.
Nurse Helpline

Wouldn't it be great to get immediate answers to your health questions, no matter what time of day or night? You can! The Avaya Healthy Decisions Nurse Helpline is a health information service that gives you toll-free access to speak with a registered nurse, 24 hours a day, seven days a week. This voluntary, confidential service is provided through SHPS, and since it's part of your health plan, it's available at no additional cost. To speak to a nurse call the Avaya Health and Benefits Decision Center toll-free at 1-800-526-8056, select “Avaya Healthy Decisions,” then choose “Nurse Help Line”. The Nurse Line is available to Avaya employees, retirees and dependents who are enrolled in an Aetna medical option for whom Medicare is not the primary payor.

Personal Health Management

Good health is your most valuable asset and Avaya wants to help you protect it. One way we can help is to make sure that you have access to appropriate screenings, treatments and medications that you need – when you need them.

The Personal Health Management Program partners with you and your doctor to help you take full advantage of your health benefits and receive the best possible care. This program compares medical claims history to national guidelines for preventive screenings and medical treatment. If any gaps are identified, we’ll let you know.

Also, if you live with a chronic disease or ongoing health condition, it can be difficult to deal with all of the issues on your own. When you participate in Avaya Healthy Decisions Personal Health Management, you won’t have to go it alone! You’ll work one-on-one with a Personal Nurse Advocate, a registered nurse who is experienced in your particular condition. You’ll get the support, information and motivation you need to take control of your health condition, plus improve your overall health.

Personal Nurse Advocates are available to assist you Monday through Friday from 9 a.m. to 8 p.m. Eastern Time. Just call the Avaya Health and Benefits Decision Center toll-free at 1-800-526-8056, select “Avaya Healthy Decisions,” then choose “Personal Health Management.”

Personal Health Management is available to Avaya employees, retirees and dependents who are enrolled in an Aetna medical option for whom Medicare is not the primary payor.
